Prescription Refills In-between Appointments:

& Schedule an appointment and make payment via phone, text or in-person.
1) Make sure you send me your (or the patient's) full name, birthday, the pharmacy name, phone number, and address, as well as the medication(s), what dose pill and how much/how many times per day you take it (for example "2mg (medication) take half tablet twice per day").
2)If you don't already have an appointment, you will need to make one for within a month.
Please allow 3 days for prescription refills:
3) Make a payment. There is a $10 charge for non-urgent (contact me to pay) refills in-between appointments (unless this is due to an error on my part for whatever reason, but not due to a lack of planning on your part) and a $20 charge for urgent refills (defined as a script needed within 48 hours: contact me to pay); I may not be able to get them to you immediately, so please plan ahead! Schedule appointments in advance and prior to medications running out.
As soon as you have sent in both the payment and the above information, I will send or call in your prescription.
While I am here to help insure that you don't run out of medications, this service is not a replacement for an appointment! I generally will not give more than a one-month supply of medications, and may only be able to give you a partial refill, depending on my clinical judgment. (This is to keep you safe, BELIEVE me that it does not make my life any easier, and any inconvenience to either one of us is to make sure that I am giving you the best medical care that I can.)
It is never a good idea to discontinue medications suddenly, the following is not a complete list, but contains some of the common medications which cause withdrawal symptoms: Effexor, Paxil, Clonazepam (Klonopin), Ativan (Lorazepam), Xanax (alprazolam, which I rarely prescribe), or Suboxone. This will most likely make you feel sick and/or can be DANGEROUS if you stop taking them abruptly. (I will add more of these meds as I think of them). Running out of stimulant medication (for example, and not a complete list: Adderall, Vyvanse, Concerta, Ritalin, etc) is not an emergency! You will not withdraw from them or experience any dangerous side effects if you do not take them on a daily basis.

"No show" policy:
Please cancel appointments online. There is a line at the bottom of the appointment confirmation email that will direct you to this link. Appointments need to be cancelled more than 48 hours in advance. This is to insure that other patients are able to schedule for an appointment that they might need, and to insure that the process runs smoothly. Any appointment where a patient needs to cancel less than 48 hours in advance for any reason, or of course, if someone does not show up punctually for their appointment, is a "no show." While emergencies do come up, I can only accommodate for these rarely. A no show fee is not billable to insurance, and is billable at $100 for a medication management appointment, $200 for a therapy appointment, and $300 for an initial evaluation. Please make sure to also leave plenty of time to wait (despite this being a double standard) in case I myself am running late. I work very hard to minimize your wait, but sometimes waits at a doctor's office are unavoidable. If you cannot wait (up to 45 minutes maximum) for your appointment, this will also count as a no-show. Conversely, in order to keep things running smoothly, you must show up on time. If you are more than 10 minutes late, I will probably not be able to see you. If you are more than 15 minutes late, it is my policy to count this as a "no show", and we will almost certainly need to reschedule. Please be punctual!
Termination of Treatment:
I am committed to being there for my patients as long as they need me. I very rarely find that I need to discontinue treatment with a patient. However this has happened rarely in the past with people who "no show" for appointments or who are lost to follow up. Please note that if you cannot follow these policies, that I would consider you to have discontinued treatment with me. For people who repeatedly do not show up for appointments, or who do not return for treatment in the general time frame recommended, this may become an issue. I do insist that people who have "no showed" pay the missed appointment fee prior to scheduling another appointment, and I will never prescribe or refill a medication unless I feel that I have seen you recently enough to do so ethically.

For Later (Evening and Night) Appointments:
These appointments tend to fill up quickly, as there is high demand for them. Please plan in advance, ie at least a week or 2, if not more. I can usually get urgent-care appointments in as "add-ons", but my available times may be very late (for both of us). Contact me if such an urgent-care appointment is absolutely necessary. If you can wait, but just need refills, I can give you a (possibly shorter supply) to get you through to my next availabilty. (Please see my policies on early refills! I often do have other evening appointments that I can schedule on Monday's and Thursday's, but these are by request only.
